Why did you not use both internal thoracic arteries?

چکیده

That is the question every cardiologist should ask if the patient he or she has referred for primary surgical coronary revascularization returns from the operating room with a single internal thoracic artery graft. There is a substantial body of evidence demonstrating that the majority of coronary bypass patients benefit more when coronary artery bypass graft (CABG) surgery is performed with both internal thoracic artery conduits rather than just one.1–4 However, only 4.4% of primary isolated CABG cases in the 2011 Society of Thoracic Surgeons National Adult Cardiac Surgery Database received bilateral internal thoracic artery (BITA) grafting.5 There are several putative reasons for this failure of adoption, but referring cardiologists and patients should demand better.

eComment: Can we use endovascular graft stenting in all type B dissections?

Authors: Mehmet Ates, Siyami Ersek Thoracic and Cardiovascular Surgery Center, Istanbul 34732, Turkey; Ibrahim Yekeler doi:10.1510/icvts.2007.169367A According to the MRI angiography, this is just a thoracic descending aortic dissection with rupture w1x. Using endovascular graft stenting should be the first choice. But, if dissection spreads toward the celiac axis, SMA and renal arteries, can w...

The role of potassium channels in the vasodilatory effect of levosimendan in human internal thoracic arteries.

OBJECTIVE We investigated the role of potassium channels in vasodilatory effect of levosimendan in human internal thoracic arteries. METHODS Samples of redundant internal thoracic arteries obtained from patients undergoing a coronary artery bypass graft surgery were cut into 3 mm wide rings and suspended in 20 ml organ baths. Isometric tension was continuously measured with an isometric force...

Differences in adrenergic nerve and receptor function in dog internal thoracic, coronary and mesenteric arteries.

Isolated dog internal thoracic arteries (ITA) responded to norepinephrine and phenylephrine with concentration-related contractions, which were suppressed by prazosin, but not by yohimbine. Clonidine did not contract ITA. In coronary arterial strips, norepinephrine produced a relaxation. Isoproterenol relaxed coronary arterial strips contracted with serotonin but did not alter the tone of ITA. ...

Bilateral internal mammary arteries: a very important missing trick for coronary artery bypass grafting.

(Oxford, UK): If what you have shown us is correct, because this is one of the longest propensity-matched follow-ups that exists in the literature, then again, as Professor Buxton showed us, this is potentially very important. But this is also the first time I have seen a paper on bilateral internal mammary arteries which showed no difference; there was absolutely no difference in deep sternal ...

Carotid angioplasty and stenting in a patient with high grade stenosis of Internal Carotid Artery associated with both vertebral arteries and contralateral carotid occlusion

Severe internal carotid artery (ICA) stenosis is a common cause of cerebrovascular accident (CVA) in middle-aged patients. Contralateral carotid occlusion (CCO) in patients with severe ICA stenosis is associated with high risk of CVA. Carotid endarterectomy (CEA) is associated with more complications in patients with CCO than those without CCO. In this study, we present the case of a 61-year-ol...
